Care Management
After a client's Solutions Plan is developed, our care managers can take the lead and set up desired services. We are not owned or operated by any other companies and do not receive compensation for referrals. This means we can ensure your loved one receives services that are most cost effective and appropriate for his or her individual needs.
Examples of Care Management Solutions may include:
Providing friendly visits to check on the client's well being.
Medical appointment management including the coordination of transportation, attending various
appointments in order to facilitate communication among doctor, client and family and helping to
ensure follow-up instructions are carried out.
Connecting clients to in-home assistance with personal care needs such as bathing, dressing, and incontinence care and assistance with home management tasks such as vacuuming, dusting and doing laundry.
Acting as a liaison for smooth transition home from the hospital or a rehab stay.
Offering support and counseling to clients and their families dealing with illness, limitations and
loss.
